As part of the development of OncoBox Breast 2.0, we are pleased to publish the sample data sets required for the first verification step of the tumour documentation systems. This marks the start of the verification process.

The first verification step requires the sample patient data sets to be entered into the tumour documentation system and then correctly exported as an XML file in accordance with the xsd schema. The XML export files are checked and evaluated by OnkoZert/ClarData. Systems that have successfully completed the first verification step are marked with a light green tick under ‘Verified Systems’.

In addition, minor corrections and additions to the version of the data field specification last published on 4th September 2025 were necessary. These changes are highlighted in colour in the updated version. The associated XSD validation schema has also been adapted accordingly.

Note for breast cancer centres: This is not an update of the existing OncoBox Breast 1.0 for the 2026 audit year, but a new development as part of the new OncoBox 2.0 generation. OncoBox Breast 2.0, which is currently under development, will be piloted next year and is expected to replace the existing OncoBox Breast 1.0 in the medium term. The update to the existing OncoBox Breast 1.0 for use by breast cancer centres for the 2026 audit year will be published in the coming weeks.

The OncoBox Lung recently marked the launch of the new OncoBox 2.0 generation. It has already been successfully used in audits in 2025. Building on this experience, the OncoBox Breast 2.0 is currently being developed and is set to replace its predecessor in the medium term. During the design and development of the OncoBox 2.0, there was an intensive exchange with experts, representatives from centres and employees of OnkoZert and ClarData, during which the new OncoBox and its operation were also presented. This showed that the OncoBox can be operated and used intuitively in many ways. Nevertheless, a certain amount of background knowledge and practical exercises are essential in order to fully exploit the potential of the OncoBox. This insight led to the creation of the ‘OncoBox driving licence’. The OncoBox licence offers interested individuals the opportunity to acquire focused knowledge and skills in using the OncoBox 2.0. This qualification is purely optional. This means that the OncoBox can also be used for certification purposes without this qualification certificate.

Following the successful piloting of OncoBox Lung as the first entity of the new OncoBox 2.0 generation, a new OncoBox Breast 2.0 will follow soon. The data field specification for OncoBox Breast 2.0 and the corresponding XSD schema are hereby published. The implementation of the XML interface by tumour documentation manufacturers can therefore already begin. OncoBox 2.0 - Newsletter 02/2025

With the update version 2.0.4 of the OncoBox Lung, enhancements have been implemented in several places and minor bugs have been fixed. An updated specification with changes compared to the previous version can be downloaded below. Only the new version 2.0.4 is to be used for audits in 2025.

Furthermore, version 2.0.4 pilots a new approach to the quality of results with the following 3 modules:

EQ Performance is intended to enable a standardized representation of the outcome quality results across all organs at calendar year level. Similar to the currently valid matrix for quality of outcome, the focus here is primarily on the follow-up rate with regard to the calendar year. This makes it possible to answer questions like: What is the current follow-up rate for cases between 2020 and 2022?

The EQ Clinical looks at the outcome quality of the case collective 1 year, 3 years, 5 years and 10 years after the case date/count date. In contrast to the EQ Performance, the evaluation time in the EQ Clinical is not the calendar year, but the periods after the case date. This makes it possible to answer questions like: How many cases have died after 3 years with an event?

The EQ indicators are intended to enable a structured evaluation of the outcome quality parameters, analogous to the existing key figure sheet, e.g. ‘How high is the overall survival after 3 years for primary cases stage III and IV’?

The new concepts for outcome quality also allow non-primary cases to be analyzed.

These new concepts are to be validated as part of the pilot program. They are not part of the certification and are not exported to the data sheet. You will find the corresponding concepts in the top menu ‘Ergebnisqualität (Pilot)’.

With the update version 2.0.2 of the OncoBox Lung, bugs in the indicator sheet were fixed and adjustments were made to the to-do list (conversion of certain deficits into notes). Furthermore, the encryption of the data export was updated for the DigiNet study. Only the new version 2.0.2 is to be used for the data delivery of the DigiNet study centres and for audits in 2025.

Furthermore, we are pleased that GTDS is the first tumour documentation system to be verified for the OncoBox Lunge. Together with a pilot centre, the system developer has successfully completed a two-stage verification process. From now on, all lung cancer centres that use GTDS for their tumour documentation can use OncoBox Lung version 2.0.2 for the audit.

The pilot phase of the OncoBox Lung begins. A beta version of the OncoBox Lung, which can be used by selected pilot centers, has been published today. If you are interested in participating in the pilot phase, please contact us. The requirement for a participation is a tumor documentation system with a completed 1st verification step (see systems with a light green tick in the “Verified Systems” tab). The beta version of OncoBox Lung includes an overall view, to-do list and case list. It also includes the calculation of the basic data and the indicator sheet for DKG certification (audit year 2024). An export of the DKG data sheet is also implemented. In the OncoBox Lung, it will be possible to document and save all reasons/causes and action taken/planned and non-calculable indicators. Note: The matrix is not yet part of the beta version and will be published in a later version.

As part of the verification process of the tumour documentation systems to the new OncoBox Lung, a beta version with partial functions is now available for the developers. The release of this version is intended exclusively for tumour documentation system developers for technical testing of their interface and not yet for use by lung cancer centers. The application can be downloaded from the "For developers" menu (tab "Lung"). The Excel-specification has also been expanded to include further relevant spreadsheets.

During the development of OncoBox Lung, it became apparent that locating the follow-up section at patient level is problematic. It was therefore necessary to restructure the system so that the follow-up is transmitted at case level and not at patient level. The adapted files (specification, xsd-schema, sample patient data sets) can be downloaded from the "For Developer" menu. Please note that the updated specification contains further 'minor' changes, which are marked in yellow/red:

• Field "Therapies distant metastases": removal of the value "Antibody therapy (AK)"
• The selection field "Name of study" has been renamed "Name of study (selection)" to ensure differentiation from the free text field "Name of study". This is merely a change in the specification; the xsd schema remains unchanged in this respect.
• The terminology of the histology codes ("Histologie Codes" spreadsheet in the specification) has been partially adapted. Furthermore, code 8013/3 was listed twice in the previous version. This has been corrected.

As part of the development and piloting of the OncoBox Lung, the following is the publication of the sample patient records that will be used for the 1st verification step in the tumor documentation manufacturer verification process. The 1st verification step for the tumor documentation manufacturers requires that the example patient records are correctly documented in the tumor documentation system and exported in the form of an XML file. A review/assessment of the XML export file is done by OnkoZert. Tumor documentation manufacturers with 1st verification step in progress are marked with a yellow tick under "Verified systems" in the "Lung" column. Tumor documentation systems with successfully completed 1st verification step are marked with a light green tick. After completion of the 2nd verification step, which involves the generation of the lung data sheet from a pilot center via the OncoBox, the tumor documentation system will be marked with a green check mark, analogous to the existing OncoBoxes.

Compared to the last published data field specification version of 22.12.2022, some corrections/additions in the data fields, XML names and explanatory texts were also necessary. These few changes are colored yellow in the data field specification and marked with red text. This also results in a modified XSD schema.

OncoBox Compare is a tool for the direct electronic comparison of data from the cancer registry and a centre based on the OncoBox "Prostate". The aim of the comparison of the data is to determine the degree of conformity and the quality and, in the case of differences, to address the causes of the deficits if necessary. The system was tested in a joint project of OnkoZert and the Cancer Registry of Rhineland-Palatinate in cooperation with two Prostate Cancer Centres from Rhineland-Palatinate. One result of the project was that the conformity of the patient collectives and data fields could be significantly increased. The project was presented at the §65c platform meeting on 22.03.2023 as well as at the DVDM focus on 20.04.2023.

The project is now to be extended to further §65c cancer registries and to further Prostate Cancer Centres. So far, four more cancer registries have registered for the "OncoBox Compare". On 04.10.2023 (3 p.m.), a web telco will take place in which the project will be presented once again in principle. Further expressions of interest will then be accepted until 31.10.2023.

Due to the new PCO subproject MID-EPIC-D and the PRO-P study, changes to the specification and the OncoBox became necessary. For PCO study centers, the OncoBox version N1.2.1 now includes the 2-year survey (overall view and patient profile). For PRO-P study centers, the study patient selection and export functionality for forwarding study-relevant data to the PRO-P-Doc study documentation system has been implemented.

The changes only affect PCO and PRO-P study centers. It is not mandatory to use this version for the 2023 audits (N1.1.1 is also fine).

Minor changes to the data field specification for OncoBox Lunge were already published on 09/08/2022. Due to further feedback, new requirements and technical restructuring, additional minor changes to the data field specification were now necessary. The changes have been implemented in this update to the specification (highlighted in yellow with red font). One restructuring that has been made is that the specific fields for the DigiNet study (first name, last name, date of birth, health insurance number) are no longer transmitted to OncoBox via the certification XML, but via a separate DigiNet XML (data field specification DigiNet XML is included in specification OncoBox Lung as a separate spreadsheet).

The XSD schema of the certification XML has also been adapted due to the changes in the data field specification, as well as the XML template and the guideline. No further changes to the data field specification are planned for the time being. The current version of the data field specification will be used to define the basic data and indicator calculations of OncoBox. To this end, we will release an update to the OncoBox Lung specification in the new year that will include these calculations.

In the framework of DigiNet (project funded by the Innovation fund), the OncoBox Lung will be developed in the upcoming years. Here, too, we are working on a first draft of the field specification. In the first step, the OncoBox Lung will be primarily used in the lung cancer centres who take also part in „DigiNet“. Tumor documentation developers who provide services to DigiNet centres will be contacted separately. The current OncoBox Colorectal was developed in 2021. Since then, only adaptions had been made. This approach came up more and more often against technological limits. Further, functional developements had been put on hold (limitation on primary cases, lack of multilinguality). Therefore, the German Cancer Society and OnkoZert have decided to deploy a new OncoBox colorectal. Right now, we are working on a first draft of the field specification. For 2022, we plan first pilots. A wide distribution of the OncoBox is planned for the year 2023. Tumour documentation developers who would like to take part actively in the framework of the pilot project OncoBox Colorectal 2.0, are invited to utter their interest. With this update, PCO study centers can now use the 1-year patient monitor. This enables the comparison of pre- and post-therapeutic survey results and thus offers prompt feedback after the post-therapeutic feedback from the patient in order to derive consequences for further treatment or to initiate internal learning processes. It is not mandatory to use this version for the audits in 2021.

Changes in the OncoBox were triggered by a global meeting of PCO data managers at the beginning of March in Los Angeles. For this version, we have implemented as far as possible the new requirements to data quality which were presented in LA.
Along with this version, you will find following changes:

• Differentiation between references on data quality (=conspicuities) and obligatory adjustment requirements (=documentation deficit).
• Automatized display of the analytical protocol which enables the centres to check the data set before sending it to OnkoZert.

This update fixes following bugs. For the calculation of transanal full-thickness resections, the second place after the comma is also allowed. Further, a conceptual failure in quality indicator no. 29 (R local, not R total, is decisive for the diagnostic report’s completeness. Finally, there was an uncertainty in exporting the EDIUM study patients. Thanks for the quick feedback. This allowed us to develop this update quickly.

Due to adaptions in the data sheet (EXCEL file) for the audit year 2018 we had to change OncoBox Colorectal. As the XML schemes have not changed, software developers do not have to adapt the interface between their tumour documentation system and the OncoBoxes. Out of the new, optional quality indicators (10, 24b, 24c, 25a, 25c, 27, 29), only no. 29 will be calculated by the OncoBox.

Further, we added additional functionalities which are necessary to manage the EDIUM study. If you import both the XML file from your tumour documentation systems (medical record) and the XML file from www.edium-studie.de (survey data), you will find additional reports in the new menu “EDIUM” (pre- and posttherapeutical return rate). In the export for the certification relevant data you will find the new spreadsheet “EDIUM-Teilnahmequote” (pretherapeutical return rate on a quarterly level). We will give more information on these new exports to EDIUM study centres separately.

Centres which do not take part at the EDIUM study, import the XML file from their tumour documentation system only and leave out importing survey data.
